Summary: This individual provides direct patient care with specific focus on the care of patients in the transition of care and post-acute care settings. The Nurse Practitioner in this role will practice according to established protocols, policies and procedures and attempted standards of care. The Nurse Practitioner will function within the approved scope and credentials as approved by Beebe Healthcare and the skilled nursing facilities of the Delmarva Post-Acute Care Network.

This Nurse Practitioner role is designed to help improve the continuity and quality of patient care in the post-acute setting, specifically skilled nursing facilities, and help to reduce avoidable readmissions.
Graduate of Nurse Practitioner program.
Responsibilities: Identify and facilitate effective strategies to improve care transitions. Assist teams to develop specific disease state care pathways that span hospitalization through the 90 day post-acute period. Collaborate with teams to ensure care pathway adherence and facilitate updated plan of care when deviation from pathway occurs. Provide consultation using assessment, diagnostic and evaluation skills to ensure early intervention of patient's clinical changes and management of clinical needs identified. Communicate effectively with providers and teams to coordinate care of patients. Provide education, coaching and counseling to patients, caregivers and team members as identified to ensure optimal outcomes. Participate in care plan meetings Participate in readmission reviews. Collaborate with patient, caregiver and teams to ensure discharge plan incorporates all key best-practice components for safe discharge to community. Conduct patient rounds with focus in the post-acute setting. Other duties as assigned.
