The Home Care Coordination Nurse Practitioner (NP) is a member of the interdisciplinary team for Home Care Coordination (HCC) Program located within the Geriatrics & Extended Care (GEC) Service. The NP is responsible to the HCC Team Physician or Medical Director for Home & Community Based Care within the GEC Service of VANCHCS. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
