The incumbent serves as a Licensed Practical Nurse (LPN) at the point of care for the Kansas City VA Medical Center, Primary Care Service's Community Based Outpatient Clinic located in Excelsior Springs, MO. The incumbent works under the direction of an RN and is part of the Patient Aligned Care Team (PACT) with a focus on patient centered care delivery.
