This hybrid position will work have a rotating schedule depending on the needs of the Heart Failure team by either working remotely to complete post heart failure discharge follow-up phone calls or working on-site at ChristianaCare as an inpatient nurse navigator with a focus on face-to-face patient education and discharge planning.
