We are seeking a Float Licensed Vocational Nurse in Geriatric Evaluation and Management (GME)/Primary Care Clinics who will provide nursing support to improve the functioning of older people with multidimensional health care needs. The nurse will participate in Comprehensive Geriatric Assessment which is evaluating the older population's functional, medical and cognitive conditions. The service model is a interdisciplinary team where they receive most of their care in the GEM clinics. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
