Typically they have a number of chronic health conditions, live in near poverty, and are too often isolated from their community and family. Position Functions & ResponsibilitiesTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Provide case management services to older adult clients living in the community; Current Case Managers will be working remotely using telehealth until otherwise notedDevelop individualized treatment plans; Coordinate needed resources; Educate clients about mental illness; Coordinate with family members and other professionals; Conduct diagnostic interviews; Provide transportation as necessary; Monitor for decompensation; Provide crisis services; The above services are primarily provided in the community, in the client's home, or in assisted living facilities. Position RequirementsKnowledge, Skills & Abilities, Education and/or Experience:Bachelor's in one of the Behavioral Sciences required. Significant experience may be substituted depending on type and nature. At least two years related experience in Social Services working with the mentally ill older adults. (Residential experience preferred.) DSHS Long-Term Care Worker training or ability to obtain upon hire required. Washington State agency affiliated counselor registration required prior to beginning direct care responsibilities. Current CPR, First Aid, and food handlers permit to be acquired within first 30 days of employment. Current Washington State Driver's License with proof of automobile insurance; ability to use own car in provision of services. #LI-JG1
