The Registered Nurse assesses, plans, implements and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction. The ICM Home Telehealth (HT) registered nurse (RN) uses the nursing process and evidence-based practice to perform nursing functions related to coordination of Veteran care utilizing patient-centered practices, and a focus on excellence in nursing care and customer service. Duties include, but not limited to: Utilization of general clinical nursing skills and knowledge in care coordination of patients and education of patients/significant others as indicated. Adhere to all current directives of VHA, and meet current Joint Commission standards, State licensure standards, and VA-OIG expectations. Perform data collection and analysis for the purpose of improving care to patient groups, maximizing use of resources to enhance Veteran outcomes. Focused on achieving optimum mental and physical health and illness prevention. The ICM/HT/RN facilitates shared decision-making with the Veteran, identifying Veteran needs/goals, developing and modifying a patient-specific plan of care, monitoring patient outcomes, and facilitating and utilizing established chronic disease management processes. Collaborating with other team members in developing the patient-centered and holistic care plan. Serving as an advocate for Veterans. Utilizing approved protocols and guidelines to authorize the provision of care, treating patients experiencing acute and chronic symptoms within the home setting. Monitoring and tracking consults in collaboration with the primary care team. Completing documentation as required. Utilizing patient education materials to optimize patient understanding regarding his or her acute and chronic disease processes and promotes the utilization of self-care skills; assuring patient understanding about when it is important to seek medical advice or attention; and, providing quality care across the continuum of life.
