Warriors Way is a ten bed neighborhood in a long-term care facility that focuses on providing safe and quality care in a home-like environment for our Veterans. Our population includes a variety of admission criteria/diagnoses ranging from needs such as social placement, dementia, rehabilitation and wound care. Our interdisciplinary team supports the Veterans with 24/7 care that align with CMS and LTCI standards and regulations while ensuring the residents have autonomy and inclusion in their activities of daily living and overall care plan. As the CLC RN you will function as a team leader in providing for the nursing care of resident Veterans. Use the nursing process to assess the needs of residents and then provide direction to the LPNs, NAs and other staff as appropriate to ensure care is provided. Some common duties of the position include, but are not limited to: providing clinical oversight of nursing care provided on the unit and is knowledgeable of regulations, VHA guidelines, unit specific policies and TJC guidelines. Function as Charge nurse, assigning staff to care for residents, ensuring adequate staffing in terms of breaks and appointments; providing medication administration using the Bar-code Medication Administration module (BCMA). Provide case management for resident Veterans to ensure that all care needs are met and that the care provided is coordinated with the multidisciplinary team. Document care provided in the Computerized Patient Record System (CPRS) in a professional manner that is consistent with policies set forth by the Central Texas Veterans Health Care System. Collaborate with multidisciplinary team to provide interventions tailored to each resident's needs to improve engagement, optimize function and decrease distress.
