This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more San Francisco VAMC is a major interdisciplinary teaching facility with service-lines serving medical, surgical, and neurological inpatients and outpatients, with a Level III Emergency Department, and multiple community-based outpatient clinics (CBOC). This Primary Care Nurse Practitioner position is at the Santa Rosa CBOC approximately 1 hour north of San Francisco. Major duties and responsibilities include, but are not limited to: Leadership in delivering and improving holistic care through collaborative strategies with others Ability to manage complex patient situations. Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care Assess, plan, implement and evaluate care based on age-specific components Consider all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences in the provision of care Prescribe and administer medications and procedures per established policies and guidelines Influence care outcomes by collaborating with members of the interdisciplinary team Assume leadership responsibilities at the patient and teamlet level Ability to recognize and respond appropriately to medical, knowledge and active participation in unit/program level quality improvement processes, and initiatives as well as customer service programs. Care is provided within the CBOC addressing preventive health care, health maintenance needs and primary care to eligible veterans Provides same day health care services and other integrative services such as nursing, pharmacy, social work, nutrition and behavioral health Back-up provider who is responsible for providing primary care to the patients of providers who are unavailable in order to offer full primary care services in the absence of the PCP and acts as surrogate to cover for provider absences for view alerts, secure messages and other needs for the absent provider's panel Work Schedule: Monday - Friday, 8:00 am - 4:30 pm (NOTE: Shifts may vary depending on patient care needs.)
