Learn more The VA Northern California Health Care System is currently recruiting for a Senior Social Worker-HUD/VASH Senior Case Manager) in the Social Work Service Line at the Oakland location. The Senior Case Manager is responsible for coordination of case management services for Veterans receiving housing provided through HUD Section 8 housing vouchers. Housing Choice Voucher (HCV) program funding will provide rental assistance under a supportive housing program for homeless Veterans authorized by section 8(o)(19) of the United States Housing Act of 1937, 42 U.S.C. 1437f(o)(19). The initiative is known as the HUD-VASH program and was authorized pursuant to Division K, Title II, of The Consolidated Appropriations Act, 2008 (Pub. Law 110-161) ("2008 Appropriation Act") enacted on December 26, 2007 (see proviso (7) under the heading "Tenant-Based Rental Assistance"). The HUD-VASH program combines HUD HCV rental assistance for homeless Veterans with case management and clinical services provided by the Department of Veterans Affairs (VA) at its medical centers and in the community. Major duties include, but are not limited to: Supervises a group of professional VA, which may include contract social workers and support staff, and provides administrative, technical, and clinical supervision necessary for accomplishing the work of the unit. The HUD/VASH program is located in multiple sites within the catchment area of NCHCS. Supervision of The Senior Social Worker is responsible for providing case management for a caseload of Veterans in the HUD-VASH program. The Senior Social Worker is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. Performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes. Verifies the Veteran's status, eligibility for VA medical care, and eligibility of program participation. Ensures that homeless Veterans are considered as primary participants for program acceptance. Collects and submits HUD-VASH Program participant data as outlined by NEPEC evaluation procedures. Provides HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing. Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Conducts psychosocial assessments to determine needs of Veterans and family members. Participates in the treatment planning process with other disciplines. Participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. Provide individual, family and group counseling as appropriate. Provide consultation/education to Veterans and families on community resources, advance directives and VA benefits.
