The Clinical Assessor possesses advance knowledge and understanding of the services and benefits available for Veterans and caregivers from both federal, state, and private sector resources. They develop and establish linkages and collaborate on VA caregiver support efforts with community healthcare and social service agencies. As a member of the interdisciplinary team, the Clinical Assessor will participate in the development and application of directed clinical treatment interventions. The Clinical Assessor participates in face-to-face or virtual contact with the caregiver and Veteran to assess function and the need for personal care services. The incumbent conducts initial and annual home visits to Family Caregivers approved in the program. Other duties include but are not limited to: Independently plans and delivers technically competent care to patients with complex needs Conducts interim and annual visits to family caregivers approved in the program Uses clinical knowledge to raise awareness of the value of caregiver issues and improve the quality and access to caregiver services Participates in direct caregiver and Veteran contacts to identify and assess caregiver stress burden Serves as a consultant to others in incorporating caregiver issues at the facility, VISN, and national levels Coordinates and integrates findings from needs assessments, inter-rater reliability assessments and program evaluations and incorporates them into learning activities Recognizes subtle abnormal signs and symptoms and/or changes in patients' condition and responds appropriately Performs other related duties as assigned Work Schedule: Monday - Friday, 8:00 am to 4:30 pm Recruitment/Relocation Incentive: Not Authorized Relocation Expenses (PCS): Not Authorized Telework: Authorized Financial Disclosure Report: Not Required *Two vacancies exist for this announcement, duty location will be determined upon selection.*
