The VA Ann Arbor Healthcare System is seeking a Licensed Practical Nurse (Telehealth Clinical Technician- Dermatology) to be a member of the Ambulatory Care Team for our new Canton, MI Community Based Out-patient Clinic (CBOC). The incumbent performs Telehealth imaging, including tele-retinal screening, Tele-Dermatology, tele-wound care, home telehealth, CVT and VVC--as identified and referred by the PCP, other providers and the imager. They will be responsible for carrying out the following duties: Provides technical and administrative support for Tele-Health programs. Works independently without direct supervision of the provider. Collects appropriate patient demographics and pertinent historical information to populate the imager template. Provides basic information and education to patients related to the imaging process. Exercises timely and appropriate action based on the consult report from the readers. Serves as a liaison between image readers, primary care providers, specialty care providers, patients and other VA personnel. Maintains certification by attending regularly scheduled QA and CE programs. Incumbent is responsible for site facilitation of Telemedicine applications which include CVT, VVC, SF and HT. They provide real-time Tele-Health support and training to providers during clinical visits. Provides information to patients, staff, and providers regarding Telehealth, Education and Training processes. Assists the remote and local provider, employee and patient as needed. Responsible for any patient, staff education/ training, documentation, and assistance with workload capture that is basic for the completion of the visit and/ or training. Opens the training, online information, and/or Tele-Health patient room. Prepares the equipment for training, education, access, exams, and establishes the online, video conferencing or other connections necessary for the visit, training or instruction. Identifies the staff, patient or provider for the presenter. Stays in the room and facilitates the Documents and closes encounters, answering questions, and performing other duties as needed to ensure re a beneficial visit, online activity and/ or training. Provides onsite technical support and troubleshooting for all telemedicine devices and program applications. Makes reminder phone calls to participants and providers or through reminder letters. Schedules the participant for follow up visits, training etc. if instructed and/or ordered. Has regular contact with the facility coordinator to work out any issues. Work Schedule: 0730-2000; 8/10/12 hour TOD may be required Functional Statement Title: Licensed Vocational Nurse (LVN) GS 6.
