Registered Nurse Care Coordinator (Outpatient Transplant, Lung)

Position description The Registered Nurse (RN) Care Coordinator works within an interdisciplinary team to coordinate care for a defined population of patients. The goals of care include supporting improvement in health outcomes, empowerment of patients and families to engage in health seeking behaviors and disease prevention, and supporting self-management of chronic conditions. Care is provided in a holistic, patient-centered approach across settings spanning the continuum of care, with an emphasis on assisting patients to seek an appropriate level of service when needed and reducing the overall cost of care The RN Care Coordinator will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for an assigned population of patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The American Academy of Ambulatory Care Nursing Scope and Standards of Practice for Registered Nurses in Care Coordination and Transition Management (2016) are reflected. Qualifications Graduate of a baccalaureate Accreditation Commission of Education in Nursing (ACEN) or Commission on Collegiate Nursing Education (CCNE) accredited nursing program with three to five years of experience. Additional qualifications Population specific experience preferred. Knowledge of population characteristics and desired outcomes. Excellent communication skills (verbal and written). Experience working in a team environment. Computer skills (e.g. PowerPoint, Excel, Word) required, prior experience with electronic medical record systems preferred. Ability to work flexible hours, which may include days, evenings, and weekends. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills. Demonstrated ability to use data for trend analysis and decision making. License or certification
